A Form Element can be considered as a single 'line' within a Form Container. It usually has a Label, and one or more elements such as Input boxes, Checkboxes, Text etc

Click on the Form Container.
Right click and select the type of Form Element to add.
- The second Form Element is now available to use. Adjust label as needed.
